As a leading provider of remote patient monitoring (RPM), chronic care management (CCM), and collaborative care management (CoCM), we prioritize proactive measures for enhanced physical health and well‑being. About the Role As a Nurse Practitioner, you will play a crucial role in delivering high‑quality preventative care to residents in skilled nursing facilities. This hybrid role combines on‑site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. You will build strong relationships with the facility, maintain a patient‑centered approach, and be a collaborative team player.
