Emergency Department Registered Nurse Care Manager
MLK Community Hospital
MLK Community Hospital seeks an RN Care Manager for our Emergency Department. This role coordinates care for diverse ER patients, ensuring safe transitions and optimal outcomes. Responsibilities include performing rapid assessments, developing care plans, collaborating with physicians and interdisciplinary teams, arranging admissions and discharges, and connecting patients with community and social resources. The RN Care Manager advocates for patients, supports families, promotes equity and culturally competent care, uses evidence‑based practices, and contributes to quality, safety, and process improvement initiatives in a fast‑paced setting. #J-18808-Ljbffr MLK Community Hospital
