Join our mission-driven team to support **individuals experiencing homelessness, mental health challenges, and substance use disorders** through case management that truly makes an impact.**Position Summary**The Case Manager I provides direct case management services to residents in Permanent Supportive Housing, using a Housing First approach and operating within an integrated behavioral and primary healthcare model.
