Responsibilities
- Manage a patient caseload and lead the interdisciplinary care team to coordinate appropriate services and resources.

- Coordinate with physicians, nurses, social work, and other professionals to ensure timely access to care and smooth transition across settings.
- Educate patients and family caregivers on treatment plans, benefits, and self-management strategies to support informed decision-making.
- Maintain accurate documentation in the medical record, monitor outcomes, and ensure compliance with regulatory standards.

Requirements
- Active Registered Nurse license in Colorado.
- Ability to work in an outpatient, coordinated care environment with a focus on case management.
- Strong assessment, critical thinking, and communication skills; experience leading a care team is preferred.
- Familiarity with care coordination processes and documentation in electronic medical records.
- Availability to work Mon-Fri 8:00 a.m.-5:00 p.m. with potential weekend backup or on-call duties.
