Position Summary

As a Nurse Care Manager, you will play a pivotal role in supporting individuals on their journey to stability and wellness. This position requires a skilled and empathetic professional with at least 3 years of nursing experience. You will collaborate with a multidisciplinary team to develop and implement care plans that address the unique needs of each individual, ensuring they receive the highest standard of care.

Key Responsibilities

• Develop and oversee individualized care plans that align with evidence-based practices.
• Provide compassionate, person-centered care to individuals and families.
• Collaborate with healthcare providers, social workers, and other professionals to ensure comprehensive support.
• Monitor and evaluate patient progress, adjusting care plans as needed.

• Maintain accurate and up-to-date documentation of care activities.
• Act as an advocate for patients, ensuring their dignity and respect are upheld.

Required Skills and Qualifications

• Minimum of 3 years of nursing experience in a relevant field.
• Strong knowledge of behavioral health, social determinants of health, and supportive care practices.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with a multidisciplinary team.
