Licensed Practical Nurse (LPN) Job Summary:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Wyomissing Health and Rehabilitation Center. As an LPN at our facility, you will play a vital role in providing high-quality care to our residents, promoting their physical, emotional, and social well-being. Licensed Practical Nurse (LPN) Responsibilities:

  • Provide direct patient care including administering medications, performing wound care, and assisting with ADLs

  • Accurately document patient information, progress, and concerns in a timely and thorough manner

  • Develop and implement individualized care plans to meet the unique needs of each resident

  • Collaborate with interdisciplinary teams to ensure seamless communication and coordination of care

  • Maintain a clean and safe environment, adhering to infection control and safety protocols

  • Provide emotional support and comfort to residents and families during challenging times

  • Current LPN licensure in the state of Pennsylvania
