Physical Therapist

Provide personalized, one-on-one physical therapy to older adults in private residences or senior living communities, supporting mobility, independence, and quality of life. Responsibilities include conducting assessments, developing care plans, delivering evidence-based treatments, documenting in EMR systems, collaborating with interdisciplinary teams, and educating patients, caregivers, and staff to promote safety and aging in place. Telehealth and virtual care may be incorporated to enhance access and flexibility. Qualifications And Skills Valid Physical Therapy license or eligibility in practice state Degree from an accredited PT program Proficiency in basic computer use and EMR systems Current CPR certification Physical Requirements Medium physical effort, including lifting up to 50 lbs occasionally Assist with patient transfers, repositioning, and therapeutic activities Extended standing, walking, climbing stairs, and driving to patient locations Ability to wear PPE when necessary The role offers autonomy, flexible scheduling, interdisciplinary collaboration, and opportunities for career growth through various employment options.
