This field-based role centers on hospital rounding, case management engagement, and discharge partnership development./ph3Key Responsibilities/h3ulliDevelop referral relationships within hospitals and acute care systems/liliConduct daily or scheduled hospital rounding/liliBuild partnerships with case management and discharge teams/liliProvide hospice education and eligibility guidance/liliRespond rapidly to referral opportunities/liliFacilitate smooth hospital-to-hospice transitions/liliTrack referral activity and conversion outcomes/liliSupport bedside consult coordination when appropriate/li/ulh3Qualifications/h3ulli3+ years hospice, home health, or post-acute liaison experience/liliStrong hospital referral background REQUIRED/liliDemonstrated census or referral growth results/liliKnowledge of hospice eligibility and benefit structure/liliExcellent clinical communication skills/liliSelf-directed and field-driven/li/ulh3Compensation/h3ulliBase + commission structure/liliMileage reimbursement/liliFull benefits package/li/ul/div
