Join Our Team as a Weekend On-Call/Admissions Registered Nurse
Are you a compassionate and dedicated Registered Nurse looking to provide patient-centered care in the comfort of patients' homes? Home Nursing With Heart in Omaha, NE is seeking a skilled RN to join our team as an Admissions Nurse, where you will play a vital role in assessing, admitting, and coordinating care for our patients. If you are passionate about delivering high-quality care with a focus on patient-centered care, we would love to hear from you.
Key Responsibilities:- Help patients with needs over the weekend
- Conduct in-home assessments to determine eligibility for home health services
- Collaborate with patients, families, and interdisciplinary teams to create individualized care plans
- Coordinate with physicians and other healthcare providers to ensure continuity of care
- Provide patient education and support to promote self-management and independence
- Ensure compliance with regulatory requirements and quality standards
- Current RN licensure in the state of Nebraska
- Previous experience in home health or hospice care preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and patient-centered approach to care
