Licensed Practical Nurse (LPN) - Work-Life Balance

Home Health Licensed Practical Nurse (LPN) – Waverly, Ohio Responsibilities Provide skilled, in‑home nursing care to patients with chronic conditions, post‑surgical needs, or other health challenges. Take vitals, perform assessments, document care, and update the RN supervisor. Support patients in following their care plan and learning self‑care techniques. Educate family members on care routines, medications, and home safety. Work alongside a care team including RNs, CNAs, and physicians. Contribute to discharge planning and ongoing care coordination. Qualifications Active Ohio LPN license. Graduate of an accredited LPN program. At least one year of LPN experience, ideally in home health. CPR certification. Familiarity with home health regulations (state and federal). Clinical confidence, strong communication, and a compassionate heart.
