Job Summary

Our client is seeking a Nurse Practitioner specializing in a Value Based Care Model, dedicated to serving the senior population by building long-lasting relationships with patients. You will work alongside a doctor, leveraging your skills in patient care, administrative tasks, and telehealth services to help reduce ER visits.

Key Responsibilities

• Prescribe medicine and manage non-complex cases.
• Ensure smooth operational flow of a full work schedule, including patient consultations and administrative tasks.
• Monitor patient health through consistent check-up appointments, scans, and preventative care.
• Adapt to patient no-shows with walk-ins, telehealth, or administrative work.
• Utilize eClinicalWorks EMR software.

Qualifications

• Strong foundation in Value Based Care, aligned with Medicare Advantage standards.
• BLS or ACLS Certification required.
• Active NP License in the state or in the process of obtaining it.
• Experience working with a geriatric population preferred.
• NP experience in areas such as Integrative Medicine, Family Practice, or Pain Management is advantageous.

Skills

• Proficiency in electronic medical record systems, specifically eClinicalWorks.
• Effective communication and teamwork.
• Capability to work autonomously with a focus on providing quality patient time.
