Registered Nurse - Case Management

Schedule

12hr Evening shift, 11:00 AM to 11:30 PM.

Assignment Details

Guaranteed Hours: 36 Weekly
Contract Weeks: 13

Requirements

• Active RN License
• 2+ years of Case Management experience
• BLS Certification required
• Care Management certification (AMC or CCM) preferred
• Must be willing to float to partner facilities as needed
• All demographic information must be fully provided at submission

Responsibilities

• Provide comprehensive case management services to patients
• Coordinate care across various departments and services
• Monitor and evaluate patient progress and treatment plans
• Act as a liaison between patients and healthcare providers
• Ensure compliance with facility policies and standards
