Overview The Home Services Social Worker assesses psychosocial needs of patients and families, providing resource support, short-term counseling, and crisis intervention in collaboration with interdisciplinary teams. Candidates must have over one year of hospice experience, strong teamwork skills, and an understanding of palliative care philosophy.
