Home Health Occupational Therapist (OT)

Location: Hood River, OR
Contract Length: 13 Weeks (Potential Extension)
Shift: Days | 5x8-Hour Shifts

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced Home Health Occupational Therapist (OT) for a contract assignment based in Hood River, Oregon. This opportunity offers a consistent weekday schedule, one-on-one patient care, and no weekend or on-call responsibilities. Both experienced travelers and first-time travel therapists are encouraged to apply. Local candidates are also welcome.

Patient Population

Common diagnoses include:

• Stroke Recovery
• Orthopedic Post-Operative Rehabilitation
• Dementia
• Parkinson’s Disease
• Multiple Sclerosis (MS)
• Amyotrophic Lateral Sclerosis (ALS)
• Other Acute and Chronic Neurological Conditions

Key Responsibilities

• Provide comprehensive occupational therapy evaluations and treatment services in patients' homes.
• Develop individualized treatment plans focused on improving independence and daily living skills.
• Perform functional assessments and home safety evaluations.
• Complete OASIS assessments and documentation accurately and timely.
• Educate patients and caregivers on adaptive equipment, home modifications, and therapeutic exercises.
• Utilize Epic EMR and mobile documentation tools for patient charting.
• Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
• Monitor patient progress and adjust treatment plans as appropriate.
• Ensure compliance with home health regulations, safety standards, and agency policies.

Required Qualifications

• Active Oregon Occupational Therapist (OT) License .
• Active Washington Occupational Therapist (OT) License .
• Minimum 1 year of Home Health Occupational Therapy experience .
• Direct OASIS documentation experience required.
• Current BCLS Certification (AHA) .
• Valid Driver's License.
• Clean Driving Record.
• Reliable Transportation.
