Activated Health & Wellness is a growing, award-winning concierge medical practice in Gilbert, Arizona, led by 8-time Phoenix Magazine “Top Doc” Dr. Melody Rodarte .

We are looking for an exceptional Nurse Practitioner or Physician Assistant who values relationships, personalized care, and taking the time to truly understand their patients.

At AHW, we believe great medicine starts with listening. Our concierge model allows our providers to develop meaningful relationships with patients while practicing whole-person medicine in a supportive, collaborative environment.

And yes — this is a Monday through Friday, daytime position. No hospital rounds. No nights. No weekends.

What You’ll Do

Provide comprehensive, office-based care for new and established patients while developing individualized treatment plans based on each patient’s goals and needs.

You’ll perform evaluations and physical exams, interpret laboratory and diagnostic testing, prescribe and manage medications, and collaborate closely with our clinical team.

Most importantly, you’ll have the opportunity to build relationships with your patients rather than simply move them through an appointment schedule .

Is passionate about patient-centered medicine, enjoys being part of a collaborative team, and wants to make a meaningful difference in the lives of their patients.

Required

• Graduate of an accredited Nurse Practitioner or Physician Assistant program

• Active and unrestricted Arizona NP or PA license

• Current unrestricted DEA registration

Preferred

• Experience in functional medicine, hormone replacement therapy, obesity medicine, or integrative health

• Certification or advanced training in obesity management

• 1–2+ years of clinical or hospital-based experience

• Strong communication skills and an excellent bedside manner

• Someone who genuinely enjoys getting to know their patients

Position Details

Job Type: Full Time or Part Time

Compensation: $60–$70/hour

Expected Hours: 20–40 hours/week

Schedule: Monday–Friday, daytime hours

Location: Gilbert, Arizona
