Care Manager - AAP - Chittenden Clinic: Work with clients and community partners as part of a Howard Center team serving individuals receiving medication for opioid use disorders through the Chittenden Clinic/HUB. Carry a full case load and provide supportive counseling and comprehensive case management based on individual client needs. The Chittenden Clinic is one of the largest medication treatment programs for opioid use disorder in the country, serving approximately 1000 patients. Our program and its staff have been recognized regionally and nationally for outstanding performance, high-quality healthcare services, and a patient-centered approach. We use evidence-based interventions and strive to reduce the adverse consequences of substance use, through a harm reduction approach, to support the health and wellbeing of all patients. Responsibilities:- Provide supportive counseling and case management
- Partner with all staff of the clinic (medical providers, nurses, administrative staff and members of drop-in team, Intake team etc) to provide person-centered care to clients
- Partner with community groups and treatment partners to coordinate client care and referrals
- Facilitate educational groups
- Provide health home services (in office) to patients as defined by the State Plan Amendment in accordance with the Affordable Care Act.
- Associates degree required or currently enrolled in associates degree program
- Apprentice Addiction Professional (AAP) certification required within 6 months of hire
- Rostering required for pre-AAP status
- Valid driver's license and use of a personal vehicle
- Full time (37.5)
- Services delivered in office setting and in community
- AFSCME Union (dues will apply)
- Non-Exempt
- Hourly rate starting at $23.68
- We offer a competitive salary commensurate to experience
- Accrue 36 days of combined time off per year for full-time employees (and increasing with years of service); pro-rated for positions 16 hours or more
