Case Manager – Bilingual (Mandarin/English)
Location: Queens, New York (Onsite)
Schedule: Full-Time
Position Summary
We are seeking a compassionate and dedicated Bilingual Case Manager (Mandarin/English) to provide assessment, care coordination, advocacy, and counseling services to geriatric clients eligible for community-based support programs. The ideal candidate will be passionate about serving older adults and helping them access essential resources and services.
Key Responsibilities
- Assess client needs and evaluate available community resources.
- Provide comprehensive case management services, including:
- Intake and assessment
- Ongoing advocacy and support
- Crisis intervention
- Information and referrals
- Financial management assistance
- In-home client assessments
- Emergency backup services
- Maintain accurate and timely client documentation and case files.
- Prepare reports, statistical data, and required program documentation.
- Coordinate client care with supervisors, nurses, and other healthcare professionals as needed.
- Establish and maintain relationships with community organizations and service providers to secure benefits and services for clients.
- Participate in staff meetings, supervision sessions, training programs, conferences, and professional development activities.
- Keep management informed of client service needs, challenges, and program activities.
- Perform additional duties as assigned.

- Complete all required annual trainings, including but not limited to:

- Compliance Training
- IT Security Awareness
- Sexual Harassment Prevention
- Adhere to organizational policies regarding attendance, professionalism, and workplace conduct.
- Support the organization's mission through excellent service, teamwork, and positive professional relationships.
Qualifications
Required
- Bachelor's Degree in Social Work or a related field such as:
- Human Services
- Public Health
- Psychology
- Counseling
- Criminal Justice
- Biology
- Social Sciences
- Minimum of 1 year of related experience in case management, social services, healthcare, or community-based programs.
- Fluency in Mandarin and English (spoken and written).
- Strong communication, interpersonal, and active listening skills.
- Proficiency with Microsoft Office Suite and general computer applications.
- Ability to manage multiple priorities while maintaining attention to detail.
- Excellent customer service skills with a commitment to treating clients with dignity and respect.
- Must be authorized to work in the United States without current or future sponsorship requirements.
Preferred
- Previous case management experience.
- Experience working with seniors, older adults, geriatric populations, or senior centers.
Work Environment
- Office-based work environment.
- Frequent use of computer, telephone, and standard office equipment.
- Occasional travel within the community for client visits and assessments.
Schedule & Interview Process
- Typical schedule: Monday – Friday, 9:00 AM – 5:00 PM.
- Alternate schedules may include 8:00 AM – 4:00 PM.
- Occasional weekend work may be required based on senior center needs.
- Employees who work weekends will receive a weekday off.
