A Medical Social Worker (MSW) in a hospital setting provides essential support to patients and their families by addressing social, emotional, and environmental factors that affect health and recovery. This role involves close collaboration with medical staff to ensure comprehensive care planning and effective discharge processes.

Responsibilities

Conduct patient assessments to identify psychosocial needs
Manage cases by coordinating with healthcare providers and community resources
Develop and implement individualized care plans
Provide crisis intervention to patients and families as needed
Facilitate discharge planning to ensure smooth transitions from hospital to home or other care settings

Preferred Qualifications

Minimum of 2 years experience in medical social work
Master's degree in Social Work (MSW)
