(1095) IDD Service Coordinator

Responsibilities The Service Coordinator position provides services to children and adults with developmental disabilities based on the Person-Centered Thinking model. Responsibilities include assessment of needs, development of service plans, facilitation of service linkages, monitoring of service effectiveness, provision of crisis intervention/prevention supports, delivery of advocacy services, advocacy and linkage for employment activities, and participation in interagency planning and CRCG meetings to bridge gaps and avoid duplication of services. The role requires frequent use of initiative and independent judgment, assertive professional intervention, frequent in‑person contact with individuals, families and service providers, and the ability to work with minimal supervision. Task list includes traveling within the service area, maintaining flexible hours and schedules, completing comprehensive assessments, service planning, referral and linkage, monitoring, advocacy on behalf of individuals served, documentation of services provided, and interaction with served individuals and families. Location & Compensation Location: Bastrop, TX Hourly Salary Range: $20.46 – $28.71 Differential Detail:

• 0.75/hr Bilingual
• 1.00/hr Human Services Bachelor’s degree
• 1.00/hr 2 years Previous Experience

Up to $4.00/hour Tenure with BTCS

• 1.00/hour – LCSW-A, LPC-A, Advanced Licensure
• 0.50/hour – LCDC

Minimum Qualifications Associate degree or higher with a major in social, behavioral, or human service field (e.g., psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice). OR a high school diploma or certificate recognized by the state of Texas as equivalent to a high school diploma, with two years of paid or unpaid experience working with individuals with intellectual or developmental disabilities. Must meet Bluebonnet Trails Community Services driving requirements and maintain approved driving status throughout employment. Preferred Qualifications At least one year of experience working with individuals with intellectual/developmental disabilities. Bachelor’s Degree. Demonstrated ability to use good clinical judgment, work effectively in a fast‑paced setting, and get along well with others. Demonstrated computer and documentation skills.
