Hospice Chaplain Requirements: Ecclesiastical endorsement by faith group/denomination Minimum of two units of Clinical Pastoral Education (CPE) from Accredited ACPE Center Required Master of Divinity degree or equivalency preferred Nonjudgmental and inclusive presence Spiritual Care experience working with terminal individuals, family members, and clinicians Knowledge and commitment to hospice philosophy of care Awareness of end of life practices associated with different faith traditions Hospice care experience preferred (one-year minimum) Excellent communication skills, both verbal and written Ability to work independently and as a member of a hospice care team Ability to use electronic medical resources for care plans, interventions, and documentation Meet federal and state requirements regarding criminal/abuse background checks Proof of valid driver’s license and reliable, insured transportation Hospice Chaplain Responsibilities: Provide or coordinate spiritual care support to patients, family members, and caregivers Conduct spiritual assessments of patients and family members in a timely manner as required Assist with bereavement follow-up care as appropriate in collaboration with bereavement coordinator Participate as a member of the hospice team in regularly scheduled care conferences, hospice admissions, and Interdisciplinary Group (IDG) meetings Provide emotional/spiritual support to hospice team members as needed Develop and maintain relationships with local spiritual leaders Participate in quality improvement programs and services Participate in weekend on-call rotation as designated #J-18808-Ljbffr Crossroads Hospice & Palliative Care
