LVN / LPN - LTC/SNF

Nursing Position

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure adherence to care plans and maintain high-quality patient care standards.

Responsibilities:

Administer medication and treatments accurately according to care plans.
Monitor residents' health status and report any changes to the healthcare team.
Document nursing assessments, interventions, and observations thoroughly.
Follow infection control protocols and maintain a safe environment.
Collaborate with interdisciplinary teams to develop and implement individualized care plans.
Comply with facility policies and state/federal regulations regarding patient care.

Qualifications:

Valid LVN or LPN license in the state of practice.
Proven experience working in LTC or SNF settings preferred.
Ability to document accurately and efficiently.
Strong communication and interpersonal skills.
Must have completed the original or updated COVID-19 vaccination or hold a valid exemption.
Ability to wear a photo badge and present a state or federal-issued ID at all times.
