About the Role: The full-time Advanced Practice Provider (APP) plays a critical role in delivering high-quality, patient-centered care across a diverse range of medical conditions. This position involves collaborating closely with physicians, nurses, and other healthcare professionals to assess, diagnose, and manage acute and chronic illnesses. The APP will be responsible for developing and implementing individualized treatment plans, performing procedures within scope of practice, and ensuring continuity of care through effective patient education and follow-up. This role demands a strong commitment to evidence-based practice, patient safety, and continuous professional development to improve clinical outcomes. Ultimately, the APP contributes significantly to the efficiency and effectiveness of the healthcare team, enhancing patient satisfaction and overall health system performance.

Minimum Qualifications:

• Master’s or Doctoral degree in Nursing (NP) or Physician Assistant Studies from an accredited program.
• Current and unrestricted Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) license
• Minimum of 5-7 years progressive clinical experience in a healthcare setting.
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.

Preferred Qualifications:

• Experience working in a multidisciplinary hospital or outpatient medical practice.
• Familiarity with electronic health record (EHR) systems such as eClinicalWorks.
• Demonstrated involvement in quality improvement projects or clinical research.
• Strong background in patient education and chronic disease management.
• Bilingual proficiency to assist a diverse patient population (English/Spanish).

Responsibilities:

• Conduct comprehensive patient assessments including history taking, physical examinations, and diagnostic evaluations.
• Develop, implement, and modify treatment plans in collaboration with the healthcare team.
• Order, interpret, and act upon diagnostic tests and laboratory results to guide patient care.
• Perform procedures and interventions within the scope of practice and institutional protocols.
• Provide patient education on disease management, medication adherence, and preventive health measures.
• Document all patient encounters accurately and timely in the electronic health record system.
• Participate in multidisciplinary team meetings to coordinate care and optimize patient outcomes.
• Stay current with medical advancements and integrate evidence-based practices into clinical care.
• Assist in quality improvement initiatives when applicable.

Skills: The required and preferred skills are integral to the daily responsibilities of the Advanced Practice Provider. Clinical assessment and diagnostic skills enable the APP to accurately evaluate patient conditions and determine appropriate interventions. Effective communication skills are essential for collaborating with healthcare team members and educating patients and their families. Proficiency with electronic health records ensures accurate documentation and facilitates seamless information sharing. Additionally, critical thinking and problem-solving skills support the APP in managing complex cases and adapting treatment plans based on evolving patient needs and emerging medical evidence.
