We offer both teletherapy and in-person therapy sessions.We are seeking a compassionate and skilled LCSW therapist to join our team as an in-home counselor to provide mental health support around issues such as mood disorders, grief, and adjustment to illness.Provide psychotherapy: Deliver individual and family therapy sessions in the person's home setting, including assisted living and independent living communities.Manage a caseload of 15-25 clients per week, depending on part-time or full-time status.Conduct thorough assessments, develop treatment plans, and monitor progress.Work collaboratively with our home-care teams, including primary care physicians, case managers, and family members.Maintain accurate and up-to-date client records in compliance with HIPAA and other regulatory requirements.Participate in scheduled community therapy sessions.Master's degree in social work (MSW) from an accredited institution.Licensed Clinical Social Worker (LCSW) in the state of Illinois.Previous experience in providing psychotherapy or social work services in the home setting preferred.Strong assessment, clinical, and communication skills.Proficiency and ease with electronic health record (EHR) systems.Willingness to travel to the region's home settings as requested.A genuine desire to help individuals improve their mental health and well-being.Flexible part-time or full-time schedule.
