Licensed Clinical Social Worker (LCSW)

We are seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our team. The ideal candidate will conduct comprehensive psychosocial assessments and develop, implement, and adjust individualized treatment and discharge plans for patients. This role involves managing a full caseload of moderate to high complexity patients, including those with co-occurring disorders and psychosocial instability.

Responsibilities

• Conduct comprehensive psychosocial assessments of patients.
• Independently develop, implement, and adjust individualized treatment and discharge plans.
• Manage a full caseload, including patients with moderate to high complexity and various discharge barriers.
• Provide crisis intervention and evidence-based therapeutic services across individual, group, and family settings.
• Lead discharge planning efforts, identifying and resolving barriers related to housing, insurance, legal status, and access to care.
• Coordinate complex transitions of care, ensuring continuity across inpatient, outpatient, and community-based services.
• Ensure compliance with regulatory, legal, and accreditation standards, including documentation requirements.
• Manage court coordination and legal processes with accuracy and timeliness.
• Maintain high-quality documentation reflecting clinical decision-making and patient progress.

Qualifications

• Master's Degree in Social Work (MSW) required.
• Licensure as a Licensed Clinical Social Worker (LCSW) is required.
• Minimum of 2 years of relevant experience preferred, including experience with travel.
• Epic experience is preferred.
• Basic Life Support (BLS) certification required.

Schedule

This is a full-time position with a daytime schedule, Monday through Friday, consisting of 8 hours per day, from 8:00 AM to 4:30 PM. Holiday coverage may be required as needed.
