Key Responsibilities:
• Conduct comprehensive psychosocial assessments to identify client needs, strengths, and housing barriers.
• Develop individualized service and housing plans in collaboration with clients.
• Assist clients in securing transitional and permanent housing, including completing applications and coordinating with housing providers.
• Provide ongoing case management, including regular meetings to monitor progress and adjust goals.
• Connect clients to community resources such as mental health services, healthcare, employment programs, and financial assistance.
• Advocate on behalf of clients with landlords, service providers, and government agencies.
• Facilitate life skills training, including budgeting, tenant rights, and independent living skills.
• Maintain accurate and timely documentation in compliance with agency and funding requirements.
• Participate in multidisciplinary team meetings and coordinate care with internal and external partners.
• Support crisis intervention and stabilization as needed.
Qualifications:
• Master’s Degree in Social Work (MSW) from an accredited program.
