Job Description Willow Brook Home is seeking a full-time MDS Coordinator to support our 50-bed care community, known for its steady leadership and collaborative clinical team. Responsibilities Completing MDS and CAA assessments thoroughly and on schedule Participating in and contributing to care plan conferences Developing and refining individualized nursing care plans based on resident needs Working closely with the interdisciplinary team to support quality outcomes Stepping in as a staff nurse as needed to stay connected to resident care This is a full-time RN opportunity for someone who takes pride in doing this work well not just completing it, but owning it.
