Nurse Practitioner, Full-Time, Day Shift

Community Care is hiring a Nurse Practitioner for our Pace Program. This is a full-time opportunity at our Racine/Kenosha and Milwaukee locations.

***$10,000 sign on bonus***

Nurse Practitioner Job Responsibilities:

• Documents and updates health history and findings, physical exam, functional status evaluations, diagnostic test and lab evaluations, mental health status and function evaluations, preventative care, and health reminders.
• Within their official scope and practice, delegates appropriate tasks to interdisciplinary team staff, and provides leadership and guidance. Acts as a resource to staff to ensure appropriate care and follow through and provides care planning assistance.
• Educates members and families regarding health maintenance and acute and chronic illnesses.
• Travel, regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Nurse Practitioner Job Requirements:

• Education: Masters degree in Nursing required.
• Licensure: Wisconsin Registered Nurse/RN license required. Family, Geriatric, or Adult NP certification is required. DEA registration required. Advance Nurse Prescriber required. NPI and CPR & AED required WI Driver License
• Experience: minimum of 1 year recent clinical nursing experience with an at risk or elderly population.

Nurse Practitioner Job Schedule:

• Monday - Friday, 40 hours per week
• 8:00AM - 4:30PM
• On-Call responsibilities: 6-7 weeks per year required
