About the Role: Solace Hospice & Palliative Care Inc is looking for a compassionate and dedicated Hospice Certified Nursing Assistant to join our caring team in Saint Charles, IL. In this meaningful role, you'll provide hands-on personal care and emotional support to patients and families during life's most important moments. If you have a heart for serving others and a commitment to dignity and comfort, we want to hear from you. Responsibilities: Provide personal care services including bathing, grooming, oral hygiene, and repositioning for hospice patients Assist with activities of daily living (ADLs) in patient homes or care facilities Monitor and report changes in patient condition, vital signs, and comfort levels to the nursing team Offer compassionate emotional support and companionship to patients and their families Assist with light housekeeping, meal preparation, and personal laundry related to patient care Document patient care activities accurately and in a timely manner Collaborate with the interdisciplinary hospice care team to ensure a holistic patient experience Requirements: Current CNA certification in the state of Illinois Prior hospice, palliative care, or end-of-life care experience preferred Strong understanding of comfort-focused, patient-centered care principles Excellent communication and interpersonal skills with patients, families, and care teams Reliable transportation and valid driver's license for home visit travel in the Saint Charles, IL area Ability to work independently with compassion, patience, and professionalism CPR certification and ability to meet health screening requirements About Us: Solace Hospice & Palliative Care Inc is a trusted provider of compassionate end-of-life care serving patients and families in the Saint Charles, IL area.
