Care Manager

Location: Central Office/Warrensburg, MO, 64093 Position Overview The Care Manager delivers individualized case management services to older adults throughout the Care Connection for Aging Services service area. This role involves assessing client needs, providing advocacy and support, screening for public benefits, maintaining accurate records across multiple data systems, connecting individuals with community resources, and assisting with programs that promote independence, health, and quality of life. The position requires extensive computer use, strong data entry skills, and regular client interaction in both office and diverse home environments. The successful candidate will demonstrate excellent communication, telephone, organizational, and problem-solving skills, as well as the ability to manage multiple priorities in a fast-paced setting. The Care Manager must maintain required professional certifications, including SHIP, SMP, and AIRS. Preference will be given to candidates with experience in community-based care management and knowledge of Medicare, public benefits programs, Options Counseling, and long-term services and supports. This role requires moderate travel within and outside the service area; therefore, candidates must possess reliable transportation, a valid driver's license, and proof of automobile insurance. The ideal candidate is a collaborative team player committed to supporting older adults and helping them remain independent in their communities. Key Responsibilities Assess client needs and develop individualized care plans. Provide advocacy and support to older adults and their families. Screen clients for eligibility for public benefits and assist with applications. Maintain accurate and up-to-date records across multiple data and case management systems. Connect clients with appropriate community resources and services. Assist with programs that promote independence, health, and quality of life. Engage regularly with clients in office settings and diverse home environments. Maintain required professional certifications (SHIP, SMP, AIRS). Manage multiple priorities effectively in a fast-paced environment. Travel moderately within and outside the service area as needed. Minimum Qualifications Bachelor's degree in social work, Special Education, Education, Psychology, Sociology, Gerontology, Behavioral Sciences, or a related field; equivalent combinations of education and relevant work experience may be considered. Strong verbal and written communication skills with the ability to effectively read, write, and convey information to diverse audiences. Demonstrated ability to build positive relationships with older adults and understand their unique needs, interests, and physical limitations. Proficiency in computer applications, including hands-on experience with data entry and case management systems. Comfortable using the Internet and Microsoft Office applications, including Word, Excel, Outlook, and other technology tools necessary to support daily operations and client services. Possess reliable transportation, a valid driver's license, and proof of automobile insurance. 2 years' experience Preferred Qualifications Experience in community-based care management. Knowledge of Medicare, public benefits programs, Options Counseling, and long-term services and supports. Ability to maintain required professional certifications (SHIP, SMP, AIRS). Strong organizational, telephone, and problem-solving skills. Ability to work collaboratively as part of a team.
