Shift Days: 8:00am-4:30pm Job Summary The Pharmacy Clinical Specialist - Pharmacogenomics Ambulatory coordinates and provides pharmaceutical care for patients within an assigned clinical area while participating in interdisciplinary rounds and collaborating closely with physicians, genetic counselors, and other healthcare providers to optimize medication therapy and improve patient outcomes. This role includes reviewing patient test results, making medication therapy recommendations, supporting pharmacogenomic‑guided treatment decisions, participating in research initiatives, and precepting pharmacy students, residents, and trainees. In addition, the specialist contributes to strategic planning and innovative pharmacogenomic projects aimed at advancing personalized patient care and expanding the integration of pharmacogenomics data within the electronic medical record. The role also provides interdisciplinary education, including didactic teaching opportunities through the Cleveland Clinic Lerner College of Medicine and affiliated colleges of pharmacy. Job Details This role is eligible for a sign‑on bonus of $10k to $15k and up to $5k relocation reimbursement. A caregiver in this role works Monday‑Friday, 8 am - 4:30 pm. No weekends or holidays. Responsibilities Perform pharmacotherapeutic monitoring and provision of pharmaceutical care for patients in the area of service responsibility in a routine, continuous manner, taking into account the age and clinical condition of each patient. Provide general and patient‑specific drug information to patients, physicians, nurses and other healthcare professionals. Interact routinely with physicians, nurses and other healthcare professionals to deliver optimal pharmaceutical care. Support the education and development of clinical pharmacists and technicians through departmental staff‑development programs and leadership in training. Assist clinical pharmacists in providing pharmaceutical care by direct interaction and formal educational programs. Participate in pharmacology‑related research within the specialty area. Maintain expertise through continual learning, journal review and other educational activity. Educate clinical pharmacists, technicians, pharmacy students, postgraduate trainees, allied health professionals, nurses and physicians in pharmacotherapy within the specialty area. Contribute to strategic planning and innovative pharmacogenomics projects to further patient care. Engage in the expansion of the use of pharmacogenomics data to guide patient care in the electronic medical record. Manage the ambulatory PGx clinic, including pre‑test counseling and results discussion with patients and caregivers, under a collaborative practice agreement. Conduct and analyze outcomes‑based PGx research and collaborate on implementation of science projects. Provide interdisciplinary education, including didactic opportunities at Cleveland Clinic Lerner College of Medicine and affiliated colleges of pharmacy. Minimum Qualifications PharmD or B.S. in Pharmacy from an accredited college of pharmacy and completion of a post‑graduate residency or fellowship training. Current State licensure as a Registered Pharmacist (RPh) or eligibility for reciprocity in the current State within 90 days of hire. Completion of a post‑graduate year 2 residency (PGY2) or fellowship. Alternatively, completion of a post‑graduate year 1 residency (PGY‑1) plus a minimum of 2 years of experience as a clinical specialist or equivalent role, with at least 50% of time spent in the specialty area, or a minimum of 4 years of such experience. For those requiring administration of immunizations: current Basic Life Support (BLS) certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved immunization course, or certification obtained within 90 days of hire. For those required to respond to cardiopulmonary arrest events: current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification through the AHA, or certification obtained within one year of hire. Preferred Qualifications Board certification. Physical Requirements Ability to stand and walk for prolonged periods, approximately 75% of the shift, and continuously for extended periods of time. Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs. Ability to reach, stretch, stoop, climb steps, and sit as required. Normal or corrected vision. Clear verbal communication skill by phone or in person. Personal Protective Equipment Follow standard precautions using personal protective equipment as required.
