Job Description
Title: Case Manager - Substance Abuse

Schedule: 8:00 AM - 4:30 PM, Sunday to Thursday

Assignment Details:
- Contract Length: 52 weeks
- Guaranteed Hours: 72 hours biweekly

Requirements:
- Bachelor's Degree in a related field preferred, or an Associate's Degree in Human Services field.
- Two to three years of experience in the area of chemical dependency preferred.
- Knowledge of community resources for continuing care/aftercare placement.
- Certified Addiction Counselor (CAC) preferred.
- Ability to complete on-site CPI and CPR training/certification.

Roles & Responsibilities:
- Maintain an individual case load, conducting intake evaluations and treatment planning.
- Complete all documentation in a timely manner as per regulations.
- Coordinate with referral sources as needed.
- Assist in retrieving patient medications.
- Support staff in crisis management and milieu management.
- Coordinate transportation needs for appointments and aftercare.
- Engage in identifying potential contraband.
- Assess patients' continuing care needs and arrange for aftercare resources.
- Participate in education and group programming.
- Attend staff meetings and provide support to family members as needed.
