Case Manager, Social Work- In Patient II

Kaiser Permanente is seeking an Inpatient Social Work Case Manager to support patients at our Washington, DC facility. In this role, you will complete psychosocial assessments, develop care and discharge plans, and coordinate services with interdisciplinary medical teams.
