Kaiser Permanente seeks a licensed inpatient Social Work Case Manager to support pediatric patients in Washington, DC. In this hospital-based role, you will perform psychosocial assessments, coordinate discharge planning, and connect families with community resources. You will collaborate closely with physicians, nurses, and care teams, leveraging digital tools and EMR systems to track care plans and outcomes. You will provide crisis intervention, counseling, and advocacy while ensuring compliance with legal, ethical, and hospital standards in a fast-paced, collaborative environment.
