Case Manager, Social Work

Kaiser Permanente seeks an inpatient Case Manager, Social Work to support pediatric patients at Washington, DC Children’s Hospital. In this role, you will conduct biopsychosocial assessments, develop care and discharge plans, and coordinate services across multidisciplinary teams. You will leverage modern digital tools to document care, track outcomes, and connect families with community resources. Working within a collaborative, innovation‑focused culture, you will provide crisis intervention, advocacy, and education while ensuring compliance with clinical, ethical, and regulatory standards.
