Role Overview As a Home Health Physical Therapist, you will assess patients' needs, develop personalized treatment plans, and deliver evidence-based interventions to restore movement, reduce pain, and improve quality of life. You will monitor patient progress, document outcomes, educate patients and families on exercises and safety, and collaborate with multidisciplinary teams to ensure cohesive, patient-centered care. Mentoring therapy assistants or students and upholding professional values are also key responsibilities. Qualifications & Skills Bachelor of Science in Physical Therapy from an accredited program Certification from the National Certification Examination for Registered Physical Therapists Valid state license to practice physical therapy Ability to work flexible hours, including weekends and holidays Valid driver’s license and insured vehicle Key Duties Evaluate patient abilities and create treatment plans Deliver interventions to promote mobility and pain reduction Monitor and adjust therapy plans as needed Educate patients and families for independence Collaborate across disciplines for comprehensive care Lead by example and mentor colleagues #J-18808-Ljbffr
