PRN Outpatient Physical Therapist/Physical Therapist Assistant

As a PRN Outpatient Physical Therapist/Physical Therapist Assistant at Highland Care Center, you will play a vital role in providing high-quality, patient-centered care to our patients. Our Outpatient Rehabilitation team is dedicated to helping patients recover from injuries, manage chronic conditions, and regain independence.
Responsibilities:
- Provide expert physical therapy and physical therapy assistant services to patients in our outpatient setting, including evaluation, treatment, and education
- Collaborate with a multidisciplinary team to develop and implement individualized treatment plans
- Conduct initial evaluations, reassessments, and progress assessments to track patient progress
- Implement and modify treatment plans based on patient response and goals
- Educate patients and caregivers on exercise programs, body mechanics, and safety precautions
- Document all patient encounters and treatment plans in a timely and accurate manner
- Participate in quality improvement initiatives to ensure excellent patient outcomes
Requirements:
- Current licensure as a Physical Therapist or Physical Therapist Assistant in the State of Utah
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Strong attention to detail and organizational skills
- Ability to adapt to changing conditions and priorities
