Dynamic Physical Therapist needed for PRN / part time visits in ALFs, home health setting. Very flexible schedule for those needing part time or PRN work. Work with a Super Star team of dedicated professionals serving an elderly population.
Responsibilities include:
- Evaluation and treatment of, but not limited to geriatric patient.
- Treatment planning for patient population.
- Supervisory visits of physical therapy assistants.
- Document all physical therapy care effectively: Evaluation, supervisory visits, maintenance visits, discharge summaries, 30 day reassessments, wheelchair evaluations to medicare standards.
- Be able to perform and use standardized testing in the home health format: MMT, 5 M walk test, Berg balance test, 30 second sit to stand, 2 minute step test and all other applicable functional measures.
- Communicate effectively with physical therapist assistant to implement effective treatment program for patients.
- Communicate clearly and effectively with team members, home health company staff, case managers, therapists, RNs, social workers, occupational and speech therapists.
- Communicate clearly with an elderly population. Keep them appraised of their current progression with physical therapy, as well as, schedule their appointments effectively.
MUST HAVE A VALID UTAH PHYSICAL THERAPY LICENSE
