Description

Conduct assessments in both inpatient and outpatient environments and create tailored treatment plans for each patient
Implement physical therapy interventions aimed at improving mobility, strength, balance, and functional abilities
Work collaboratively with nurses, physicians, case managers, and other rehabilitation experts to ensure the best outcomes
Provide education to patients and their families regarding therapeutic goals, transition planning, and at-home exercises
Record evaluations, treatments, and progress in line with facility policies and regulatory requirements
Engage in care team discussions and interdisciplinary meetings for coordinated patient care
Education

~ Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution

Certifications

State licensure as a Physical Therapist
Preferred: CPR certification
Additional specialized certifications (e.g., in geriatrics or neurology) are a plus but not mandatory
Skills

Prior experience is strongly encouraged
Ability to treat patients with intricate medical and mobility challenges
Strong teamwork and communication abilities
Comfort with digital documentation and clinical charting systems
