Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments, creating targeted treatment plans tailored to individual needs
• Provide physical therapy interventions that address mobility, strength, balance, and overall function
• Engage collaboratively with nurses, physicians, case managers, and other rehabilitation associates to maximize patient outcomes
• Guide patients and their families through therapeutic objectives, discharge protocols, and home activity strategies
• Maintain detailed records of assessments, treatment approaches, and progress following hospital requirements and regulatory standards
• Participate actively in care teamwork and interdisciplinary meetings to ensure smooth transitions and comprehensive care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification is preferred
• Specialty certifications such as neurological or geriatric therapy are advantageous but not essential

• Prior experience in the field is greatly valued
