Nurse Practitioner, Optum Care at Home - Saint Louis County, MO $20,000 Student Loan Repayment Or $10,000 Sign-on Bonus This position requires daily travel to patient homes within assigned area. The Optum at Home program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of patients (beneficiaries) in their place of residence. The APC within OAH provides care to our highest-risk health plan beneficiaries and is part of an interdisciplinary team. Primary Responsibilities: Past medical history review, medication reconciliation, vital signs, and physical exam Evidence-based screenings and point-of-care testing (as appropriate) Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions Ensure accurate and complete documentation, including ICD 10 conditions Communicate and collaborate with the interdisciplinary care team Conduct advanced illness and advanced care planning conversations Provide patients and caregivers with counseling and education You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care Active, unrestricted licensure in Missouri (or ability to obtain by start date) without current disciplinary actions or practice limitations Ability to gain a collaborative practice agreement, if applicable in your state Ability to navigate varied environments; ability to sit/stand/kneel as needed to perform duties Active and unrestricted driver’s license and access to reliable transportation Preferred Qualifications: 1+ years of clinical experience as an APC (family, geriatric, or home health preferred) Experience in geriatric medicine, long-term care, senior living or home care setting Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs Understanding of Geriatrics and Chronic Illness Understanding of Advanced Illness and end of life discussions Proficiency with electronic medical records and technology Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
