Advanced Practice Provider (APP) I, CareLine The Advanced Practice Provider (APP) I, CareLine is responsible for diagnostic patient care primarily through virtual, remote consultation via video conference or telephone. The incumbent serves as the dispositional authority for after‑hours and weekend clinical calls and is accountable for assessing, diagnosing, treating and precisely documenting patients’ physical and psychosocial health status. Schedule: Mon: 17:00‑21:00 Fri: 17:00‑21:00 Sat: 10:00‑22:00 Sun: 11:00‑22:00 Responsibilities Through virtual video conference or telephone, assess acute and non‑acute clinical problems. Perform and document physical evaluations and patient histories, analyze trends, and develop, document and implement patient management plans. Aid in the development of care plans that may include health education, physician referrals, case‑management referrals, and patient/family counseling. Plan patient care based on knowledge of the patient population and protocols, considering cultural background, level of understanding, personality, and support systems. Act as patient advocate and collect comprehensive data related to patients’ health needs. Analyze data to determine appropriate maintenance and improvement methods. Confere with the patient’s PCP and other providers to report health data and ensure guideline compliance. Ensure achievement of optimal patient outcomes through telemedicine and collaborate with on‑call PCP as needed. Implement appropriate protocols and evaluate progress toward expected outcomes. Work with key contributors to enhance telehealth practices and systems using data on program effectiveness. Communicate using a variety of formats, tools, and technologies to build professional relationships across the continuum. Utilize resources to plan and provide services that are safe, effective, and financially responsible. Provide exceptional customer service and professionalism to all internal and external customers. May participate in clinical rounds, risk and quality management programs, and relevant meetings. Adhere to departmental goals, performance standards, regulatory compliance, quality patient‑care policies, and procedures. Practice in accordance with a written or electronic practice agreement. Participate in the formulation of telehealth policies, procedures, and protocols. Initiate or participate in quality‑improvement activities that achieve approved outcomes. Participate in committees that support growth and provide feedback on practice improvement. Coordinate services with other programs and perform other duties as assigned. Knowledge, Skills, and Abilities Adherence to state laws regarding travel for onsite observation and medical‑record review as required (Georgia, Tennessee, and other applicable states). Expert-level business acumen and understanding of core job-related functions and practices. Comprehensive knowledge of medical practices for independent certification and collaboration with licensed physicians and other providers. Consistently achieve clinical performance metrics. Technical capability to conduct telemedicine visits in accordance with state and federal regulations. Excellent clinical judgment, problem‑solving, prioritization, and work‑under‑pressure skills. Ability to provide constructive feedback and collaborate professionally with physicians, patients, and team members. Effective operation within multidisciplinary teams. Proficiency in Microsoft Office Suite and other required systems. Willingness to travel up to 10% of the time and to attend meetings and training in states that require onsite observation. Availability to work evenings, overnight, weekends, and four holidays per calendar year. Fluency in English; bilingual in Spanish or Creole is a plus. Ability to exercise independent judgment and obtain autonomous practice as an NP in applicable states. Ongoing acquisition of knowledge and skills to maintain expertise in the area of practice. Education and Experience Criteria Bachelor’s degree in Nursing (BSN) and graduate of a school of nursing for Advanced Practice Nursing with required specialty certification. Master’s degree in Nursing required. Board certification by AANP or ANCC required. Basic Life Support (BLS) certification from the American Heart Association or the American Red Cross required upon hire. Multi‑state licensure to include FL, VA, and at least two of GA, MI, MO, OH, PA, TN, TX, IL, KY, LA. Minimum of 3 years of acute/primary‑care clinical experience. Preferred 2 years of telehealth experience. Salary Pay Range: $111,140 – $158,771. Final compensation depends on experience, education, geographic location, and other relevant factors.
