Key Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans
• Provide direct physical therapy services in a skilled nursing and rehabilitation setting
• Document patient progress and adjust treatment plans as appropriate
• Collaborate with interdisciplinary care team to support patient recovery goals

Required Qualifications

• Active Physical Therapist (PT) license in the state of Texas
• Graduate of an accredited Physical Therapy program
• Experience working in a skilled nursing or rehabilitation environment

Preferred Experience / Skills

• Prior experience in a long-term care or post-acute rehabilitation setting
• Familiarity with Medicare documentation and compliance requirements
