This role is focused on Home Health Care in the area surrounding Dearborn Heights and Allen Park with a steady referral source from multiple skilled nursing facilities. This role includes PTA supervision and provides the opportunity to pick up additional time at a nearby SNF. What is Take Your Therapist Home? In a world of shorter and shorter SNF stays, Take Your Therapist Home (TYTH) is an unique program where clinicians work directly with patients in the SNF and home health providing a continuity of care allowing for you to have a greater impact on patient outcomes! Rivetus has been serving the geriatric Post-Acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians. Our perspective is unique in providing continuity of care across the health care continuum in the Skilled Nursing, Home Health, and Outpatient settings. Benefits:

- Physical Ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, without restriction, utilizing lifting devices clinically recommended for the patient.
