Job Overview Patient Navigator Nurse – act as the central hub of a multidisciplinary care team, advocating, interpreting, coordinating, educating, supporting, and counseling patients as they progress through their care journey. Provide clinical expertise in specialty care management, and serve as the critical connection and liaison for patients recovering from strokes, cancer, and long‑term illnesses. Responsibilities Provide written and verbal communication that links patients, families, physicians, physician staff, and other team members regarding the patient’s medical treatment plan. Coordinate treatment plans across internal and external departments and agencies to ensure continuity of care. Assist patients and families in understanding diagnoses, treatment options, and available resources. Review medical documentation to help physicians obtain complete and accurate records. Serve as the essential link between patients and all other care providers. Follow patients throughout the care continuum, collaborating with care‑management resources and facilitating appointments for consults, social work, PT, OT, ST, dietitians, physicians, and financial resources. Collect data, track outcomes, and support strategic planning processes. Plan, implement, and evaluate patient care from screening through survivorship, demonstrating knowledge, skill, and coordination. Assist patients and family members in problem solving, education, and decision support related to treatment. Review and update policies related to service provision when appropriate. Serve as a resource for community educational events such as health fairs, screenings, symposiums, and lectures as well as for staff education. Qualifications Current nursing license to practice in Louisiana as defined by the Louisiana State Board of Nursing. American Heart Association BLS certification. At least 3 years of professional nursing experience in a specialty area.
