Work Environment

The Family Nurse Practitioner will operate primarily onsite, conducting visits within the Salem community. The typical work schedule spans from 9:00 a.m. to 6:00 p.m., with the potential for occasional evening clinics extending until 8:00 p.m. Participation in community outreach events and health fairs may occasionally require weekend availability, fostering a deep connection with the local community.

Key Responsibilities

• Conduct thorough health assessments and screenings, taking a comprehensive approach to evaluate the physical, emotional, and social needs of patients.
• Diagnose and manage both acute and chronic health conditions, implementing preventive care strategies and health promotion activities tailored to individual patient needs.
• Collaborate with community organizations and stakeholders to design and execute effective health education programs that cater to the unique demographics of the Salem area.
• Provide culturally sensitive and empathetic patient care, championing patient rights and ensuring they have access to necessary healthcare services.
• Facilitate referrals and coordinate follow-up care to guarantee continuity and quality of care across various health service providers.
• Maintain accurate, detailed, and legally compliant medical records utilizing electronic health record systems and ensuring best practices in documentation.
• Engage in community outreach initiatives by participating in health screenings and educational events to foster awareness and promote healthier lifestyles.
• Stay informed about current trends in community health, evidence-based practices, and relevant policy changes to continuously improve service delivery.

Patient Population / Client Focus

Qualifications

• Current certification as a Nurse Practitioner is required.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 per year, commensurate with experience and qualifications. In addition, our comprehensive benefits package includes:
