Position Overview

Pulse Point Internal Medicine is seeking a dedicated and compassionate Nurse Practitioner to join our growing team in Salem, New York. This full-time role focuses on providing high-quality, value-based primary care to adults, especially those enrolled in Medicare. As a Nurse Practitioner, you will play a critical role in ensuring our patients remain healthy, independent, and out of the hospital by delivering personalized, empathetic care in a community-focused setting.

Work Environment

This position is primarily onsite, involving direct patient interaction within the home care setting. You will be part of a multidisciplinary care team that includes a supervising physician, other Nurse Practitioners, physician assistants, medical assistants, registered nurses, and care managers. Together, this collaborative team works towards shared goals of primary care excellence and population health management. Key Responsibilities

• Provide exceptional primary care services, conducting thorough assessments during office visits for both routine and acute medical issues.
• Administer annual wellness visits and perform health risk assessments, emphasizing holistic approaches to health and detailed documentation.
• Coordinate patient care effectively with other healthcare providers, specialists, and associated agencies to enhance care continuity and outcomes.
• Lead population health initiatives, ensuring necessary screenings and preventive care are accessible and delivered to all eligible patients.
• Assist your care team with phone triage and outreach, proactively addressing patient concerns and facilitating access to care.
• Educate patients on their health conditions and treatment plans, fostering a better understanding of their health journey.
• Participate in organizational initiatives, including promotional activities that align with our mission and vision.
• Conduct home visits as necessary to provide care in the patient's environment.
• Other responsibilities as required, reporting directly to the Medical Director and collaborating closely with operational leadership.

Patient Population / Client Focus

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completed an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through recognized accrediting bodies such as AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license and a DEA license.
• Experience in primary care settings such as internal medicine, geriatrics, or family medicine is essential.
• A solid understanding of HCC documentation, ICD-10 coding, and conducting health risk assessments.
• A collaborative mindset and a passion for teamwork across various functions within the organization.
• Bilingual proficiency is a plus and can enhance patient engagement.

Preferred Experience

• Prior experience in home care or similar settings is highly desirable, as it can help in understanding the unique challenges of delivering care in the community.
• Familiarity with electronic health records and other relevant healthcare software can improve workflow efficiency.
• Experience leading health initiatives or improvement projects within a clinical team context.

Compensation and Benefits

We offer a competitive salary range of USD 180,000 to 200,000 annually, commensurate with experience and expertise. Additionally, our benefits include:
