RN Case Manager - Sign-On Bonus & Discharge Planning

Alta Hospital is seeking an RN Case Manager to perform clinical assessments and reassessments for utilization review, discharge planning and patient throughput. You will coordinate care with the interdisciplinary team and advocate for patient self-determination and safety. The role requires CA RN licensure, strong communication, and experience in acute care case management.
