Key Responsibilities\n

• Conduct detailed assessments of seniors’ psychosocial needs, integrating emotional, mental, and social factors into a comprehensive evaluation.

• Collaborate with clients and their families to develop personalized care plans that cater to individual goals, preferences, and identified challenges.

• Provide counseling and emotional support to seniors, assisting them in navigating transitions, whether related to health, family dynamics, or living situations.

• Advocate for clients’ rights and facilitate access to vital community resources, ensuring they receive the necessary services to promote their welfare.

• Work with a multidisciplinary team, ensuring continuity of care through effective communication and collaboration on client progress.

• Regularly monitor client well-being through virtual or in-person visits, assessing changing needs and potential risks, and ensuring a safe living environment.

• Facilitate family communication, helping to resolve conflicts regarding care and ensuring all family members are informed and on the same page regarding the client’s needs.

• Educate clients and families about available community resources, support groups, and programs that can enhance their quality of life and social engagement.

• Maintain accurate and timely documentation of client interactions, assessments, and care plans using electronic health record (EHR) systems.

• Keep informed about current best practices in social work and comply with relevant policies and ethical standards to provide the highest quality of care.

Patient Population / Client Focus\n

In this role, you will primarily focus on senior clients, many of whom may experience complex psychological and social challenges related to aging. You will engage with clients dealing with various life challenges—be it physical health issues, financial concerns, or emotional distress—and work alongside their families to foster environments that promote mental and emotional health.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current and valid LCSW licensure in New Jersey.

• At least two years of experience in a social work role, ideally within a community center or geriatric setting.

• Exceptional assessment, counseling, and interpersonal skills necessary to establish a strong rapport with clients and families.

Compensation and Benefits\n

The annual salary for this LCSW role ranges from USD 60,000 to 100,000, depending on experience and qualifications.
