Schedule: 8 hours per day
Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 32
Requirements:
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- CHPN certification - Preferred
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program
- Bachelor's of Science in Nursing - Preferred
Roles & Responsibilities:
- Assess and respond to patients' and families' needs in various settings.
- Develop, implement, document, and evaluate individualized Plans of Care (POC).
- Provide education to patients and families about care, medications, and home safety.
- Supervise and guide home health aides to ensure continuity of care.
- Ensure compliance with third-party payer requirements for assigned cases.
- Communicate effectively with healthcare team members and serve as a liaison for patients and families.
