Key Responsibilities

• Conduct comprehensive mental health assessments, integrating observations of patients’ physical health and co-occurring medical conditions that may influence their mental well-being.
• Develop and implement tailored treatment plans that incorporate both mental health and medical care, aligning with best practices in integrated care.
• Facilitate individual and group therapy sessions using evidence-based therapeutic methods, such as cognitive-behavioral therapy (CBT) or dialectical behavior therapy (DBT), to promote personal growth and symptom management.
• Collaborate actively with a range of medical providers—such as primary care physicians and psychiatrists—to ensure seamless coordination of care and improved treatment outcomes for patients.
• Provide timely crisis intervention and support during mental health emergencies, utilizing effective de-escalation techniques.
• Offer educational resources to patients about the connections between mental health and physical health, empowering them to make informed decisions about their care.
• Document all patient interactions accurately and confidentially, ensuring compliance with legal and ethical standards.
• Engage in team meetings, case reviews, and interdisciplinary care planning to share insights and enhance care strategies.
• Stay informed about current research, best practices, and advancements in integrated mental health care to continuously improve service delivery.

Patient Population / Client Focus

Our therapeutic services primarily focus on individuals facing complex mental health issues, including those with co-occurring medical conditions. The ideal candidate will demonstrate a commitment to understanding the nuances of patients’ experiences, particularly those who encounter multiple layers of challenges, such as anxiety, depression, chronic pain, or other significant health concerns.

Qualifications

• A Master’s or Doctoral degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
• Current and valid licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Psychoanalyst, Psychologist, or Licensed Marriage and Family Therapist (LMFT) in New York.
• Demonstrated experience working with mental health issues in patients with co-occurring medical conditions.
• Exceptional interpersonal and communication abilities, fostering a strong rapport with patients and families.
• Adept at maintaining professional boundaries while providing empathetic and confidential support.
• Proficient in using electronic health record systems and technology pertinent to clinical practice.
• A strong collaborative spirit, thriving as part of an interdisciplinary healthcare team.

Preferred Experience

Preference will be given to candidates with prior experience in an outpatient setting, particularly those who have developed integrated treatment plans for individuals facing both psychological and medical challenges. Familiarity with telehealth platforms and virtual therapy approaches is highly desirable as we adapt to the evolving landscape of mental health care delivery.

Compensation and Benefits
